SMSNA Fellowship in Advanced Prosthetic Urology and Male Sexual Health Program Structure

The Duke University Fellowship in Advanced Prosthetic Urology, Male Sexual Health, and Reconstructive Surgery is a one-year clinical and research fellowship endorsed by the Sexual Medicine Society of North American (SMSNA).

The fellowship is designed to provide fellows with comprehensive postgraduate training in the disciplines of male sexual medicine, genitourinary prosthetics, male stress urinary incontinence, Peyronie’s Disease, cancer survivorship, and urethral stricture disease.

This is an operatively heavy fellowship where fellows will average 4 days per week in the operating room. In addition to high case volumes we offer wide-ranging research opportunities and substantial research infrastructure. Overall, the SMSNA fellowship at Duke University offers a transformative educational experience, providing fellows with the resources, mentorship, and expertise needed to become leaders in the field of sexual medicine and prosthetics.

Medical and Surgical Treatments

Penile Prostheses

  • Primary and revision penile implant surgery (inflatable and malleable)
  • Management of complex cases: corporal fibrosis, prior infection, erosion, and/or altered anatomy (end-stage implant patients)
  • Implant salvage techniques and infection prevention protocols

Male Incontinence

  • Artificial urinary sphincter (AUS) placement and revision
  • Male sling procedures
  • Evaluation and management of neurogenic and post-prostatectomy incontinence

Genitourinary Reconstruction

  • Urethral stricture disease: anastomotic and substitution urethroplasty
  • Genitoplastics: buried penis repair, scrotoplasty, perineal reconstruction
  • Management of traumatic and iatrogenic injuries to the anterior urethra and external genitalia

Male Sexual Medicine

  • Erectile dysfunction: medical therapy, penile duplex ultrasound, intracavernosal injection therapy
  • Peyronie’s Disease: in-office treatments as well as surgical correction including plication and plaque excision/incision with grafting

Scrotal and Testicular Disorders

  • Microsurgical varicocelectomy
  • Microsurgical spermatic cord denervation for chronic orchialgia
  • Testicular prosthesis placement
  • Management of hydrocele, spermatocele, epididymal cysts, and scrotal lymphedema

Andrology and Cancer Survivorship

  • Evaluation and treatment of male hypogonadism, including testosterone replacement therapy and alternative regimens
  • Fertility preservation and collaboration
  • Management of ejaculatory disorders and low libido
  • Lifestyle optimization and preventive men’s health interventions
  • Sexual and urinary rehabilitation following prostate, bladder, and rectal cancer treatments
     

Didactic Training

Fellows in the Duke SMSNA Fellowship benefit from a robust didactic curriculum designed to complement their clinical and surgical training. Educational opportunities include:

  • SMSNA Fellowship Bootcamp: A comprehensive, foundational course on male sexual dysfunction, hosted by the Sexual Medicine Society of North America
  • Industry-Sponsored Training Courses: Hands-on opportunities to engage with new technologies and devices in prosthetic urology and male incontinence
  • Virtual Grand Rounds: Participation in SMSNA-sponsored lectures featuring national experts in sexual medicine and reconstructive urology
  • Duke Urology Educational Programs: Including the Fridays in Urology symposium series, Departmental Grand Rounds, and journal clubs
  • Daily Teaching: In-depth learning through intraoperative instruction, inpatient rounds, outpatient clinic discussions, and formal and informal case reviews
